RESOLUTION AWARDING CONTRACTS
TO ST. LUKE'S OCCUPATIONAL MEDICINE FOR MEDICAL SERVICES IN THE AMOUNT
RESOLVED, that St. Luke's
Hospital Occupational Medicine be and hereby are awarded contract for furnishing
medical services for the administrative services division in accordance
with specifications on their proposal and unit cost for a total of $70,000,
terms net 30, FOB destination, payable out of various funds, depts./agencies,
organizations and objects; to be encumbered out of year 2001 budget.
OF PURPOSE: This resolution will authorize the purchase
of medical services from St. Luke's Hospital Occupational Medicine who
has submitted a proposal to provide medical services as specified.
The St. Luke's Hospital Occupational Medicine specified prices for specific
services and this arrangement allows for flexibility in matching vendors
with services needed by the city at any particular time (i.e., employments
physicals, drug screens, emergency treatment, fitness for duty exams).
When the city needs these services, it will consult the guaranteed prices
and service descriptions we now have from St. Luke's Hospital Occupational
Medicine. This resolution authorizes flexible procurement of medical
services as needed by the city. There is sufficient budget appropriation
to award these contracts; to be encumbered out of year 2001 budget.
