Water or Gas Meter ApplicationINITIAL CONTACT: Customer Service Division, Application Desk, 520 Garfield Ave., Duluth MN 55802. FORM: Application for Water and/or Gas Meters. FEE: See Fee Schedule. TELEPHONE NUMBER: (218) 730-4050. ISSUED TO: Any individual desiring to purchase a commodity of water and/or natural gas for their use in a building or dwelling. PROCEDURE: Individual must sign an application for meter and have their credit approved which may include a security deposit payable in advance. Order is then written for service division to set meter. For heating customers a heat loss card must be on file with the engineering division prior to the setting of gas meter. There is no fee for setting meters, customer pays only for volume of water or gas used and recorded on the meter and billed to the customer. Sewer user charges are based on water meter readings. All brackets or connections for meter installation are to be installed by owner's plumber. The department installs only the meter.
